Surrogacy Miracles Family Foundation Application

APPLICATION INFORMATION & REQUIREMENTS 

Thank you for your interest in the Surrogacy Miracles Family Foundation grant program. This application is quite detailed and will necessitate providing thorough information regarding your financial and medical background. Additionally, some forms must be downloaded and completed by a physician. We require this information to ensure we can make the most informed decision.

IMPORTANT NOTICE: Surrogacy Miracles Family Foundation recognizes that certain information and forms needed for this application contain sensitive medical and financial details. GRANT:

• FET (Fetal Embryo Transfer) Costs include:
  • FET Clinic Fees
  • Monitoring Expenses
  • Medication Costs
  • Travel Expenses
  • Totaling up to $20,000
• Gestational Carrier Compensation:
  • Compensation as per the fully executed Gestational Service Agreement
  • Totaling up to $20,000

The deadline to submit your fully completed application is 11.15.2024.  Grant winner will be announced 12/10/2024. 

A nonrefundable application processing fee of $75 is required for grant consideration. Payment must be submitted before your application can be processed. Any notifications regarding delays in deadlines or announcements will be communicated via email to all applicants.

ELIGIBILITY REQUIREMENTS: Please review these requirements carefully, as failing to meet ANY of them could result in an automatic denial of your application.

• Surrogacy must be medically necessary (medical conditions listed here)
• Must be a citizen or a legal permanent resident of the United States (U.S.)
• Must reside in one of the following states: GA, MD, FL, NC
• Must be working with one of these IVF clinics: Hope Fertility, Shady Grove, Atlanta Center for Reproductive Medicine, Reproductive Biology Associates
• Must be affiliated with a Surrogate Agency located in the US
• Gestational surrogate must reside in the U.S
• Embryos must be created (and frozen) at the time of application submission
• Euploid (genetically normal) embryos must be generated and ready to use.
• Must be open to sharing your story publicly
• Must use Seed Trust Escrow Management for Surrogacy Journey.

REQUIRED ATTACHMENTS: You will be able to upload the required documents below on the last page of the application.  

PERSONAL STATEMENT: Allow us to understand you and your circumstances. Your personal statement should cover the following points (maximum of two pages):

• Share details about yourself and your family
• Provide a summary of your medical need for surrogacy
• How winning this grant will help impact our life and surrogacy journey
• Include any other information you believe we should be aware of

MEDICAL: Please have your Reproductive Endocrinologist (Fertility doctor) complete and sign the medical form. Please allow sufficient time for your physician to complete these pages and return them to you as it is needed for the completion of your application.
